Your physician or other provider (together "Provider"), through suppliers to your Provider, makes this and related websites (all together, the "Website") available for your use. If you use this Website, request a consultation, or purchase or pay for products or services, you are a user and agree to these Terms. You must be at least eighteen (18) years of age.

2. Consultations and Services

Booking a free SoftWave consultation through the Site is a request to schedule a visit with a licensed provider. It does not create a treatment plan or guarantee that SoftWave is appropriate for you. Appropriateness for any treatment is determined by a licensed provider during an in-person consultation. This website does not provide medical advice.

3. Communications

You may receive email and phone communications related to your inquiry or consultation. SMS and text messages are sent only if you check the optional SMS consent checkbox. Using this Site or providing a phone number does not constitute consent to receive text messages.

SMS/Text Messaging

Starwood Med Spa may send you SMS/text messages only if you affirmatively check the optional SMS consent checkbox. Providing a phone number or submitting a form without checking that box does not constitute consent. Using this website does not constitute SMS consent. Messages may include inquiry responses, SoftWave consultation scheduling, appointment confirmations, reminders, and visit updates.

6. Cancellation Policy

Starwood Med Spa requires a 24-hour notice to cancel or reschedule an appointment. A $50 No Show fee may apply as described in our Spa Policies, including for late arrival of more than ten minutes.

7. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Texas. Venue for disputes in connection with these Terms shall be in Dallas County, Texas.
